Post-Hospital Follow-Ups

Continuity of care after discharge. We come to you to review medications, wound care, and recovery progress.

Chronic Disease Check-Ins

Regular visits for homebound patients managing diabetes, hypertension, COPD, and other ongoing conditions.

Multilingual Visits

Mobile visits available in English, Spanish, Croatian, and Swahili — wherever you are most comfortable.

Mobile visits start at $200

Pricing includes travel. Final cost may vary based on distance and services provided. Self-pay — we provide a superbill for insurance reimbursement.
